Physical Therapist (Sign-On Bonus)

About the Role

We are seeking licensed Physical Therapists to provide exceptional, one-on-one care to older adults in their homes and senior living communities.

Responsibilities

• Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy to support mobility, independence, and wellness.
• Document patient progress accurately within electronic medical records.
• Collaborate with a multidisciplinary team including PTs, OTs, SLPs, and fitness specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Incorporate telehealth and virtual care into treatment when appropriate.

Requirements

• Valid Physical Therapy license and degree from an accredited program.
• Proficiency with computers and EMR systems.
• Current CPR certification.
• Ability to meet physical demands such as lifting, repositioning patients, and assisting with transfers.
