Physical Therapist (RELOCATION/SIGN ON BONUS)

About the role

We are seeking a licensed Physical Therapist to provide personalized, in-home and senior living care to older adults.

Responsibilities

• Performing evaluations and developing individualized treatment plans
• Delivering evidence-based therapy to enhance mobility, independence, and quality of life
• Documenting care effectively within electronic medical records
• Collaborating with an interdisciplinary team of PTs, OTs, SLPs, and fitness professionals
• Educating patients, caregivers, and staff to promote safety and aging in place
• Providing telehealth and virtual care to extend access and flexibility

Qualifications

• Valid Physical Therapy license and degree from an accredited program
• Proficiency with computers and EMR systems
• Current CPR certification

Skills and physical requirements

• Ability to exert moderate effort, assist with patient transfers, and perform therapeutic activities
• Willingness to drive to patient locations and wear PPE as needed
