Physical Therapist (RELOCATION/SIGN ON BONUS)

Role: Physical Therapist Responsibilities: Provide one-on-one physical therapy to older adults in private residences and senior living communities, perform evaluations, develop and implement personalized treatment plans, support patient wellness and independence, document care in EMR, collaborate with an interdisciplinary team, educate patients and caregivers, and incorporate telehealth when appropriate. Qualifications & Skills: Valid PT license, degree from an accredited program, proficiency in EMR and basic computer skills, current CPR certification, and ability to meet physical demands such as lifting and assisting patients, performing CPR, and driving to patient locations. Key Duties: Deliver evidence-based therapy, promote safety, support aging in place, and maintain effective documentation.
