About the Role
Licensed Physical Therapist providing in-home, senior living, and virtual care to older adults.
Responsibilities
- Conduct evaluations and develop individualized treatment plans
- Deliver evidence-based therapy to enhance mobility and independence
- Document care in EMR and collaborate with an interdisciplinary team
- Educate patients and caregivers
- Utilize telehealth when appropriate
- Provide one-on-one therapy in various settings (home, senior living, virtual)
- Support patient wellness and safety, promote aging in place
- Adjust treatment schedules to meet patient needs
Requirements
- Valid Physical Therapist license
- Degree from an accredited Physical Therapy program
- Proficiency with EMR and computers
- Current CPR certification
- Ability to meet physical demands such as lifting and patient assistance
