Physical Therapist (RELOCATION BONUS)

About the role

Seeking a licensed Physical Therapist to deliver personalized, one-on-one care to older adults in private homes and senior living communities. The role involves conducting evaluations, developing individualized treatment plans, and providing evidence-based therapy to enhance mobility, independence, and quality of life. Therapists may incorporate telehealth and virtual care to increase flexibility and patient access.

Responsibilities

  • Provide in-home and community-based physical therapy sessions
  • Perform patient evaluations, create care plans, and deliver treatments
  • Document care within electronic medical records in compliance with standards
  • Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists
  • Educate patients, caregivers, and staff on safety and aging-in-place strategies
  • Utilize telehealth when appropriate to extend care access

Qualifications & Skills

  • Valid Physical Therapy license and degree from an accredited program
  • Proficiency with computers and EMR systems
  • Current CPR certification

Physical Requirements

  • Ability to exert physical effort, including lifting up to 50 lbs occasionally
  • Assist with patient transfers and repositioning
  • Perform therapeutic activities, stand, walk, and climb stairs
  • Drive to patient locations and wear PPE as needed
