Physical Therapist – Rehabilitation Services

About the role

We are seeking a dedicated Physical Therapist to provide outpatient rehabilitation services in-home for the older adult population, primarily in assisted and independent living facilities and community settings.

Responsibilities

• Assess patients and develop and implement individualized therapy programs to restore and enhance physical performance.
• Educate and train patients and caregivers to promote independence and well-being.
• Make referrals to other disciplines as needed and collaborate with the rehabilitation team to ensure comprehensive care.
• Supervise support personnel, maintain documentation, and adhere to treatment schedules.
• Perform therapeutic exercises, manual therapy, functional training, and use modalities to support recovery.
• Design, fit, and instruct on adaptive devices, including orthotics and prosthetics.
• Conduct home and community mobility assessments and promote health and wellness.

Qualifications

• Valid Physical Therapy license in the practice state or proof of license eligibility.
• Master’s degree in Physical Therapy or related field, or a Bachelor’s degree with five years of progressive PT experience.
• Reliable transportation or a valid driver’s license.
