Physical Therapist – Rehabilitation Services

About the role

The Physical Therapist provides outpatient rehabilitation services in-home for older adults, assessing and developing personalized therapy programs to promote independence and improve function. Supervise support personnel, ensure timely documentation, and participate in team meetings.

Responsibilities

• Evaluate patients through history, tests, and measures to establish diagnoses and treatment plans.
• Implement therapeutic techniques such as exercise, manual therapy, modalities, and functional training.
• Design and fit adaptive devices; instruct patients and caregivers on mobility and safety.
• Perform reexaminations, modify treatment plans, and incorporate health and wellness programs.
• Conduct mobility assessments and provide recommendations for safe community and home mobility.

Qualifications

• Valid state license or eligibility
• Master's degree in Physical Therapy or related field (or equivalent experience)
• Reliable transportation
