Responsibilities

• Provide outpatient rehabilitation services in an in-home setting for the older adult population, including private residences and independent living facility rooms.
• Evaluate and treat patients in their most functional environment, facilitating rehabilitation through individualized therapy programs.
• Assess the need for and deliver physical therapy programs to restore, reinforce, and enhance performance.
• Identify and refer patients to other disciplines to address comprehensive needs.
• Instruct, educate, and train patients and caregivers in skills essential for promoting independence and productivity.
• Direct activities to promote and maintain health, including health and wellness programming.
• Perform home and community mobility assessments and provide recommendations for safe mobility.
• Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
• Ensure adherence to department and patient treatment schedules, timely clinical and billing documentation, and work-related meetings.
• Attend and contribute to patient care, staffing conferences, and related meetings.
• Consult with other members of the rehabilitation team to select the most appropriate therapeutic regimen.
• Adapt programs and plans of care according to individual patient needs.
• Promote a safe environment and enforce the elimination of fire and safety hazards.
• Order supplies and equipment as necessary; clean, maintain, identify, and report the need for repair of equipment.
• Screen, examine, and evaluate patients, including history, systems review, and application of appropriate tests and measures.
• Establish diagnoses, identify impairments, determine predicted level of improvement, and design and plan of care.
• Develop treatment goals and methods in collaboration with the patient and caregivers; implement the physical therapy treatment plan; complete all related documentation.
• Apply therapeutic techniques including therapeutic exercise, balance/coordination training, gait/locomotion training, neuromotor development, relaxation, strength/endurance training, manual therapy, airway clearance, integumentary repair, electrotherapeutic/physical agent modalities, and mechanical modalities.
• Design, fabricate, fit, and/or apply adaptive devices; train patients in their use.
• Select activities suited to the individual’s physical and mental capacity, intelligence level, and interest.
• Instruct patients in skills and techniques required for participation in mobility activities; evaluate progress, attitudes, and behavior related to rehabilitative potential.
• Perform routine reexamination as needed/required to modify/progress the plan of treatment.
• Perform other related duties as required.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
• Master’s degree in Physical Therapy; or Master’s degree in a Physical Therapy related field plus a Bachelor’s degree in Physical Therapy; or Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience.
• For degrees earned outside the U.S., must have an evaluation confirming U.S. equivalency to a degree in Physical Therapy.
• Valid driver’s license or reliable, independent transportation to and from patient residences.

Pay

Salary range: USD $80,000.00 – USD $110,000.00 per year.

Bonus

USD $5,000.00 signing bonus.
