Travel Physical Therapist job in Hyannis, MA focused on home health. You’ll help patients regain mobility and independence through personalized rehabilitation plans. Responsibilities

• Evaluate patient needs and set measurable treatment goals.
• Implement and modify personalized therapy programs.
• Document patient progress and outcomes.
• Collaborate with an interdisciplinary home health team.
• Educate patients and caregivers on rehabilitation plans.

Requirements

• 1+ years of Physical Therapist experience in home health.
• Active Massachusetts PT license (PT-MA).
• BLS certification from the American Heart Association (AHA).
• Bachelor’s degree or higher in Physical Therapy from an accredited program.
• Willingness to work occasional holidays and 2 weekends (or 4 weekend days) within a 13-week contract.
• SSN and date of birth required for compliance.

Pay

Pay rate: $2,359.00 - $2,431.00 per 8-day shift.
