About the role
Travel Physical Therapist (PT) Home Health jobs in Jackson, MI are available for 13-week contracts. Home Health experience is preferred, but candidates with less experience may be considered for longer contracts. You will provide one-on-one patient care in home settings, develop personalized treatment plans, and document visits using Homecare Homebase EMR. Work independently to support patients’ mobility and recovery goals.

Requirements
- Michigan Physical Therapist license
- Home Health experience preferred
- 1 professional reference covering entire work history
