About the role

Deliver personalized physical therapy to patients in their homes, helping them regain strength, mobility, and independence. Assess and document patient progress, develop rehabilitation plans, and provide clear instructions to PTAs and Home Health Aides.

Requirements

• Current and unrestricted Ohio physical therapy license
• CPR certification
• Experience collaborating with healthcare teams
• One year of physical therapy experience (preferred)
• Compact license accepted
• Work history with no more than 30 days gap in the last 7 years
• 1 professional reference from the last 12 months

Pay

$2,567.00 - $2,645.00 per shift.
