Physical Therapist - Home Health - (PT - HH)

About the role

Deliver direct patient care, evaluate functional needs, design and implement individualized plans, and document progress to support patient recovery.

Responsibilities

• Evaluate functional needs and design individualized plans of care
• Implement and modify treatment plans based on patient progress
• Document progress and communicate with interdisciplinary team members
• Provide education to patients and families about their condition and treatment plan
• Collaborate with other healthcare providers to ensure comprehensive patient care

Requirements

• At least 1 year of home health experience
• VA license
• Rapid response credentialing available within 7-10 days

Qualifications

• Physical Therapist, Home Health PT(Compact Accepted)
• Certifications: BLS

Skills

• Effective communication skills
• Strong clinical judgment and problem-solving abilities
• Ability to work independently and as part of a team
• Knowledge of home health care protocols and standards

$2388.00 - $2461.00 per week
