The Home Health Physical Therapist opportunity is based in Roseburg, Oregon, a welcoming community known for its scenic beauty and family-friendly lifestyle.

Responsibilities

• Provide skilled physical therapy services to adult and geriatric patients in their homes throughout Roseburg and the surrounding area.
• Perform comprehensive evaluations and develop individualized plans of care.
• Deliver hands-on treatment focused on improving functional mobility, strength, balance, and safety in the home environment.
• Educate patients and caregivers on home exercise programs, fall prevention strategies, safe transfers, and the proper use of assistive devices.
• Manage a community-based caseload that may include patients recovering from surgery, stroke, cardiac events, fractures, and those living with chronic conditions such as COPD, heart failure, or neurologic diagnoses.
• Coordinate closely with the rest of the care team to ensure patients receive comprehensive, integrated services.
• Travel between homes within a defined geographic territory around Roseburg, allowing you to become familiar with the community while minimizing extensive driving.

• Physical Therapist, Home Health
• Experience: 1 year
• Licenses: PT(Compact Accepted), SSN Required, DOB Required
• References: 1 Reference in entire work history
