Physical Therapist - Home Health - (PT - HH)

About the role

As a Home Health Physical Therapist, you will provide one-on-one, evidence-based physical therapy services to adult and geriatric patients in their homes throughout Salisbury and surrounding communities.

Responsibilities

• Perform comprehensive physical therapy evaluations in the home
• Identify functional limitations, home safety risks, and patient-centered goals
• Develop and implement individualized treatment plans focused on gait training, balance and strengthening programs, transfer and stair training, fall prevention strategies, pain management, and education on energy conservation and safe mobility
• Assess the need for assistive devices and durable medical equipment and provide training to both patients and caregivers on proper use
• Work closely with an interdisciplinary home health team, collaborating with nursing, occupational therapy, speech therapy, and medical social work to coordinate care, update plans of care, and support smooth transitions from hospital or facility to home
• Ensure clear, timely electronic documentation in the agency’s EMR system

• Physical Therapist, Home Health
• Experience: 1 year PT(Compact Accepted)
• SSN Required
• DOB Required
• References: 1 Reference in entire work history
• Driver's License and Auto Insurance Required
