Physical Therapist - Home Health - (PT - HH)

About the role

Provides physical therapy services in patients' homes.

Responsibilities

• Assesses patients' physical condition and develops treatment plans.
• Administers therapeutic exercises and treatments to improve mobility and strength.
• Makes regular visits to patients' homes to monitor progress and adjust treatment plans as necessary.
• Communicates with patients, families, and other healthcare providers to ensure coordinated care.

Requirements

• Physical Therapist license required.
• Home Health PT license preferred.
• SSN required.
• DOB required.
• References from at least one previous employer.

Qualifications

• At least 2 years of clinical experience in physical therapy.
• Knowledge of home health care protocols and standards.
• Excellent communication and interpersonal skills.

Skills

• Strong clinical judgment and problem-solving skills.
• Ability to work independently and manage time effectively.
• Proficient in using medical software and equipment.

Pay

$2419.00 - $2493.00 per shift.
