Responsibilities
- Performing diagnostic tests and measurements
- Developing and implementing individualized care plans
- Documenting visits using Homecare Homebase EMR
- Educating clients and their families on rehabilitative care and activities to promote health, safety, and independent living
Requirements
- Physical Therapist, Home Health PT(Compact Accepted)
- Home Health experience preferred
- Active Vermont license required
- Strong organizational and interpersonal skills essential
