About the Role
Provides in-home physical therapy services to patients with neuro and/or musculoskeletal disorders, under the direction of a physician.
Responsibilities
- Perform assessments and develop treatment plans
- Schedule patient visits in a timely and efficient manner, checking in with the office daily to update schedule
- Work independently and handle emergencies without assistance
- Drive in various conditions, follow written or verbal instructions, and read maps
- Participate in weekend and on-call rotation
- Maintain regular contact with agency staff, physicians, hospital staff, and the general public
- Handle confidential information appropriately
Requirements
- Graduate of an accredited physical therapy program
- Current state physical therapy license
- Valid driver’s license
- CPR certification
- Good physical health with the ability to lift up to 50 pounds frequently and occasionally lift over 50 pounds
- Strong assessment, treatment planning, time management, and problem-solving skills
- Excellent communication and documentation skills
- Ability to adapt to change, take initiative, and set challenging objectives
- Willingness to learn and grow professionally
Preferred Qualifications
- Two years of experience in acute care or home care
