Responsibilities

• Perform initial and ongoing assessments to determine level of functioning, including OASIS assessments at appropriate time points.
• Perform initial and interim assessment and evaluation of the client’s level of functioning.
• Identify problems and set goals.
• Develop the plan of treatment in consultation with the physician.
• Provide physical therapy in accordance with the plan of treatment.
• Revise the plan of treatment as needed.
• Coordinate interdisciplinary care and adhere to the plan of care.
• Provide patient/family education as appropriate.

Qualifications

• Graduate of a physical therapy curriculum approved by the American Physical Therapy Association.
• Licensed by the State of Michigan.
• One year of clinical experience preferred.
• Must be vaccinated or have lab‑confirmed immunity for measles, mumps, rubella, and varicella; must receive an annual flu vaccine (or approved medical/religious exemption).
