Physical Therapist - Home Care

Role Overview

Seeking a Physical Therapist to provide personalized 1:1 rehabilitative therapy in patients' homes and communities. They will conduct comprehensive assessments, develop treatment plans, and deliver therapy tailored to individual needs while evaluating and adjusting care plans as needed. The role involves practicing independently within an interdisciplinary team to support patient recovery from surgery, illness, or injury.

Qualifications & Skills

• Minimum of one-year physical therapy experience or successful completion of a rehab intern probationary period
• Licensure to practice as a Physical Therapist in New York State
• Valid driver's license required outside Manhattan

Key Duties

• Assess patient needs and develop treatment strategies
• Provide personalized rehabilitation therapy in patients' homes or facilities
• Monitor patient progress and modify plans accordingly
