Physical Therapist - Home Care (Bilingual)

About the role

Seeking a Physical Therapist to provide personalized, 1:1 rehabilitative therapy within patients' homes and communities. The role involves independent practice as part of an interdisciplinary team, conducting comprehensive assessments, developing treatment plans, and adjusting care based on patient progress. The therapist will deliver care to patients recovering from surgery, illness, or injury, ensuring their needs are met through ongoing evaluation and plan modifications.

Responsibilities

• Engage with patients in their homes and communities
• Conduct comprehensive patient assessments

• Provide effective rehabilitation therapy tailored to individual conditions

Requirements

• Minimum of one year of physical therapy experience or successful completion of a probationary period as a Rehab Intern
• Licensure in New York State
• Valid driver’s license for areas outside Manhattan
