Physical Therapist - Home Care (Bilingual)

About the role

We are seeking a Physical Therapist to provide personalized, independent rehabilitative therapy in patients' homes and communities. You will assess patient needs, develop treatment plans, and adjust care based on evolving conditions as part of an interdisciplinary team.

Responsibilities

• Conduct comprehensive evaluations
• Deliver tailored therapy
• Monitor patient progress

Requirements

• Minimum one year of physical therapy experience or successful completion of a probationary period as a Rehab Intern
• Licensure to practice as a Physical Therapist in the state
• Valid driver’s license required outside Manhattan
