Physical Therapist - Home Care (Bilingual)

Overview

We are seeking Physical Therapists to provide personalized, independent rehabilitative care in patients' homes and communities. They will conduct comprehensive assessments, develop treatment plans, and adjust care based on patient progress. Therapists work as part of an interdisciplinary care team to support recovery from surgery, illness, or injury.

Responsibilities

• Delivering tailored therapy sessions in patients' homes or care facilities
• Assessing patient needs and creating individualized treatment plans
• Monitoring and modifying care plans as patient needs evolve

Qualifications

• Minimum one year of physical therapy experience or successful completion of a probationary period as a Rehab Intern
• Licensure to practice as a Physical Therapist in New York State
• Valid driver's license required outside Manhattan
