About the role
We are seeking Physical Therapists to provide personalized, one-on-one rehabilitative therapy in patients' homes and communities. They will assess patient needs, develop treatment plans, and adjust care based on evolving conditions, working independently within an interdisciplinary team.
Responsibilities

- Monitoring progress
Qualifications
- Minimum one-year physical therapy experience or successful completion of a probationary period as a Rehab Intern
- Licensure to practice as a Physical Therapist in New York State
- Valid driver's license required outside Manhattan

- Strong assessment and treatment planning abilities
- Excellent communication and responsiveness to patient needs
- Ability to work independently and as part of a team
