Physical Therapist - Home Care

About the role

This position involves providing personalized physical therapy services in patients' homes and community settings, assessing patient needs, developing treatment plans, and adjusting care based on evolving conditions. The therapist works independently within an interdisciplinary team to deliver rehabilitative care, ensuring optimal recovery outcomes.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized treatment plans.
• Deliver tailored therapy programs in home or care facility settings.
• Monitor patient progress and modify care plans as needed.
• Practice independently while collaborating with healthcare team members.

Qualifications & Skills

• Valid New York State Physical Therapy License and current registration.
• Bachelor’s Degree in Physical Therapy from an approved program.
• Minimum of one year of physical therapy experience or completion of a rehab internship.
• Basic PC skills; bilingual abilities may be preferred.
