Physical Therapist - Home Care

Overview

We are seeking a Physical Therapist to provide personalized, independent rehabilitative therapy in patients' homes and communities. They will assess patient needs, develop treatment plans, and adjust care based on evolving conditions, collaborating with an interdisciplinary team.

Responsibilities

• Conduct comprehensive assessments
• Deliver tailored therapy programs to support recovery from surgeries, illnesses, or accidents
• Deliver personalized therapy in patients' homes or care facilities
• Evaluate patient progress and modify care plans accordingly
• Practice autonomously within an interdisciplinary team setting

Qualifications

• Minimum of one-year physical therapy experience or successful completion of a probationary period as a Rehab Intern
• Valid New York State License and current registration in Physical Therapy
• Valid driver's license required outside Manhattan area
