Physical Therapist - Home Care

Job Summary Provides evaluation and physical therapy treatment in compliance with state and regulatory standards. Primary Responsibilities Analyzes patient responses, documents progress, revises care plans, and recommends updates based on interventions.Establishes and documents patient and family education plans, communicates effectively with patients and healthcare team, and schedules visits.Implements and adjusts treatment plans collaboratively, utilizing assessments like goniometry, muscle testing, and sensory evaluations.Independently manages care for routine to complex cases, maintains accurate clinical documentation, and submits reports timely.Participates in case conferences, multidisciplinary teams, community outreach, and continuous education.Performs assessments to determine rehabilitation needs, recommends adaptive equipment, and plans discharge and community referrals.Performs modalities such as therapeutic exercises, gait training, and heat/cold therapy. Qualifications Bachelor’s degree in Physical Therapy from an accredited program.1-2 years clinical experience.Valid Physical Therapist license, valid driver’s license, and CPR certification required.Strong clinical, organizational, communication, and computer skills, with knowledge of home care regulations.
