Physical Therapist- Home Care
About the Role
This opening is for the Home Health Float Team, based out of Roanoke, providing coverage to surrounding areas. Mileage reimbursement, company equipment, and a sign-on bonus are available.
Responsibilities
- Confirm and interpret physician referrals, perform patient assessments, and plan/implement individualized treatment programs accounting for developmental/chronological age and medical diagnoses.
- Provide patient care based on physician referral and age-appropriate assessment, designing treatment programs to achieve goals set with the patient and/or family.
- Coordinate patient care through written/verbal communication with patients, families, therapists, Clinical Team Leaders, managers, physicians, and other healthcare team members.
- Document in the patient's medical record and complete other care-related forms per regulatory and department/hospital policies.
- Supervise Physical Therapist Assistants (PTAs) and collaborate with therapy staff, support staff, volunteers, and students to ensure efficient clinical activity and quality care.
- Participate in departmental/professional activities including in-services, performance improvement, program development/evaluation, new employee orientation, committee membership, peer review, and staff meetings.
- Deliver excellent customer service and patient care with respect, dignity, kindness, and empathy; champion the organization's mission, vision, and values.
- Orient new employees and provide guidance to volunteers, students, support, or contract staff.
- Provide problem-solving, consultation, advising, or interpretation services to others as needed.
Qualifications
Education: Graduate of an approved school of Physical Therapy. Bachelor's degree as a Physical Therapist with 5 years of experience, OR a Master's or Doctorate degree is required.
Licensure/Certification: Licensed to practice in the Commonwealth of Virginia by the Virginia Board of Physical Therapy. AHA BLS certification (from a Carilion approved program) must be obtained within six months of employment.
Experience: For Home Health positions, one year of clinical experience is required. Experience in site-specific level of care is preferred but not required.

- Knowledge of life span growth and development principles; ability to assess and interpret age-specific patient needs and provide care accordingly.
- Within scope of license and per physician referral, interpret orders, perform assessments, and plan/implement treatment programs.
- Physical ability to lift, manipulate, push, pull, bend, and walk with patients (may weigh over 200 pounds).
- For Home Care: must provide own transportation and travel daily between patient homes.
- Ability to work at other Carilion facilities as needed.
Pay
$39.00 - $62.40 per hour. Employee pay is based on qualifications, credentials, education, and prior work experience.
