Physical Therapist - Home Care

About the role

We seek a dedicated Physical Therapist to provide personalized 1:1 rehabilitative therapy in patients' homes and communities. The role involves conducting comprehensive assessments, developing tailored treatment plans, and evaluating patient progress to adjust care as needed. You will practice independently within an interdisciplinary team, delivering therapy to support recovery from surgery, illness, or injury.

Responsibilities

• Assess patient needs and develop treatment plans
• Deliver personalized therapy in home or care settings
• Monitor and modify plans based on patient progress
• Work independently within a team environment

Qualifications

• New York State Physical Therapy license and registration
• Bachelor's Degree in Physical Therapy from an approved program
• Minimum one year of experience or successful completion of probationary period
• Basic PC skills; bilingual abilities may be preferred
