About the role

We are seeking a Physical Therapist to deliver personalized, independent rehabilitative therapy in patients' homes and communities, collaborating with an interdisciplinary care team.

Responsibilities

• Conduct comprehensive assessments to determine patient needs and develop tailored treatment plans.
• Deliver individualized therapy sessions and evaluate patient progress.
• Adjust care plans based on evolving patient needs and responses.

Requirements

• Minimum of one year of physical therapy experience or successful completion of probation as a Rehab Intern.
• Current NY State license and registration in Physical Therapy.
• Valid driver’s license required outside Manhattan.
