Physical Therapist - Home Care

About the role

Seeking a Physical Therapist to deliver personalized, 1:1 rehabilitative therapy in patients’ homes and communities. You will conduct comprehensive assessments, develop treatment plans, and adjust care based on patient progress, working as part of an interdisciplinary team.

Responsibilities

• Practice independently in the community.
• Assess patient needs and develop tailored treatment plans.
• Provide hands-on therapy and monitor patient progress.
• Respond to evolving patient needs with care plan adjustments.

Qualifications

• Minimum of one-year physical therapy experience or successful completion of probation as a Rehab Intern.
• Licensed to practice as a Physical Therapist in the state.
• Valid driver’s license required outside Manhattan.
