Physical Therapist - Full-Time and Part-Time

About the role

We are seeking a licensed Physical Therapist to provide one-on-one care to older adults in private residences and senior living communities, supporting mobility, independence, and quality of life through our signature Geriatric House Calls™ model.

Responsibilities

• Delivering therapy sessions to enhance patient wellness, mobility, and functional independence
• Documenting care using our EMR system in compliance with regulatory standards
• Collaborating with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Incorporating telehealth and virtual care to increase access and flexibility

Qualifications and skills

• Valid state license or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Proficiency with basic computer and EMR systems
• Ability to meet physical demands of in-home and community care, including patient transfers and driving
