Physical Therapist - Early Supports and Services
About the role
Lakes Region Community Services is seeking a Physical Therapist (PT) for Early Supports and Services to join our Family Resource Center team. This is a part-time opportunity with flexibility to become a full-time position.
The position includes assessing as part of a trans-disciplinary team to determine current development level and eligibility for Early Intervention services. Additionally, the PT will develop and implement an Individualized Family Service Plan with family and other appropriate individuals.
The right candidate will have experience working with young children and their families in the community, be comfortable with family-centered approaches, be committed to working in a team environment, and be flexible with scheduling. Experience with and knowledge of Early Supports and Services is preferred.
Responsibilities
- Assess children as part of a trans-disciplinary team to determine development level and eligibility for Early Intervention services.
- Develop and implement an Individualized Family Service Plan with families and other appropriate individuals.
Requirements
- Doctor of Physical Therapy (DPT) degree.
- License from the NH Office of Professional Licensure and Certification.
- 1-2 years of professional experience working with young children and families (preferred).
- Valid driver’s license, reliable vehicle, and automobile insurance.
Pay
$36.00 per hour.
