About the role

As a Physical Therapist at Family Life Care, you will play a vital role in managing the care of our patients by providing compassionate and professional care in the comfort of their own home. FLC serves our entire community from children to senior citizens. You will have the opportunity to choose from various assignments, such as patient case manager, long-term care, or assisting with recovery from an injury or illness. FLC works with you to build your work schedule around your needs.

Responsibilities

• Patient assessment, admission, and coordination of care with other disciplines, physicians, and care managers
• Provide Physical Therapy and treatment of home care patients in accordance with the plan of treatment and with company policies and procedures
• Deliver Physical Therapy in a pleasant, professional manner to maximize positive outcomes and patient satisfaction
• Ensure all clinical assessments, care plans, starting, and resumption of care are completed within agency guidelines and deadlines
• Train, coach, and direct clinical staff in the performance of their clinical duties
• Responsible for compliance with all state and federal laws, regulations, policies, and processes relevant to home health

Qualifications

• Current Florida Physical Therapy license
• Current CPR Certification
• Driver’s License and Vehicle Insurance
• Current TB test with negative results
• Ability to pass a Level II background check

Skills

• Positive, approachable, and mentoring personality
• Understanding of state and federal laws and regulations related to home health, especially Medicare and Medicaid
• Experience with Medicare (OASIS), Medicaid, and commercial insurance in the home health environment is desired
• Strong interpersonal and communication skills, organizational abilities, and attention to detail
• Experience with industry software such as Electronic Medical Records (EMR) and Electronic Visit Verification (EVV) systems
