About the role
Licensed Physical Therapist providing personalized, one‑on‑one care to older adults in private residences and senior living communities, utilizing the Geriatric House Calls™ model.
Responsibilities
- Conduct evaluations and develop individualized treatment plans.
- Deliver evidence‑based therapy to improve mobility, independence, and quality of life.
- Document care accurately in the EMR.
- Collaborate with interdisciplinary teams.
- Educate patients and caregivers.
- Incorporate telehealth when appropriate.
Key Duties
- Provide in‑home and community‑based physical therapy services.
- Perform assessments and create tailored treatment plans.
- Support patient wellness, mobility, and functional independence.
- Educate patients, caregivers, and staff on safety and aging‑in‑place strategies.
- Utilize virtual care tools to enhance accessibility and continuity.
Qualifications
- Valid state physical therapy license.
- Degree from an accredited physical therapy program.
- Current CPR certification.
- Ability to meet physical demands such as lifting, repositioning patients, and driving to various locations.
Skills
- Proficiency in electronic medical record (EMR) systems.
