About the role

Licensed Physical Therapist providing personalized, one‑on‑one care to older adults in private residences and senior living communities, utilizing the Geriatric House Calls™ model.

Responsibilities

• Conduct evaluations and develop individualized treatment plans.
• Deliver evidence‑based therapy to improve mobility, independence, and quality of life.
• Document care accurately in the EMR.
• Collaborate with interdisciplinary teams.
• Educate patients and caregivers.
• Incorporate telehealth when appropriate.

Key Duties

• Provide in‑home and community‑based physical therapy services.
• Perform assessments and create tailored treatment plans.
• Support patient wellness, mobility, and functional independence.
• Educate patients, caregivers, and staff on safety and aging‑in‑place strategies.
• Utilize virtual care tools to enhance accessibility and continuity.

Qualifications

• Valid state physical therapy license.
• Degree from an accredited physical therapy program.
• Current CPR certification.
• Ability to meet physical demands such as lifting, repositioning patients, and driving to various locations.
