About the role
Physical Therapist providing personalized in‑home and senior‑living care to older adults, utilizing evidence‑based treatments to improve mobility, independence, and quality of life.
Responsibilities
- Conduct patient evaluations and develop individualized care plans.
- Deliver therapy sessions that support wellness and functional independence in private residences and senior‑community settings.
- Document all care activities in the electronic medical record (EMR).
- Collaborate with an interdisciplinary team of healthcare professionals.
- Educate patients and caregivers on treatment plans, home exercises, and safety.
- Incorporate telehealth to extend access and maintain continuity of care.
- Ensure patient safety, mobility, and aging‑in‑place through personalized care.
Qualifications & Skills
- Valid Physical Therapy license (or eligibility) and degree from an accredited program.
- Proficiency with computers and EMR systems.
- Current CPR certification.
- Ability to meet physical demands such as lifting, repositioning patients, and driving to patient locations.
