About the role
The West PACE Physical Therapist plays a crucial role in the PACE program, focusing on restorative and rehabilitative Physical Therapy services for participants. This position requires a minimum of two years of experience in a clinical setting with a frail or elderly population, along with a valid Physical Therapy license in California.
Responsibilities
- Conduct Physical Therapy assessments to determine care needs, preferences, and goals for participants.
- Participate in IDT meetings to develop and document care plans, including interventions such as exercise, muscle re-education, gait training, and ADLs.
- Deliver treatments like massage, heat, sound, water, light, or electricity as per physician orders.
- Supervise Therapy Assistants, providing training, instruction, and guidance.
- Order, track, and manage maintenance for therapeutic equipment.
- Update the IDT promptly regarding changes in participants' condition or functional status.
- Collaborate with the Occupational Therapist to run the rehabilitation department efficiently.
- Work with other team members to ensure smooth care transitions between different care settings.
- Support enrollment of prospective participants into the program.
- Participate in end-of-life care coordination and support.
- Complete required documentation accurately and in a timely manner.
- Protect privacy and maintain confidentiality of all company procedures and information.
- Follow West PACE policies and procedures and engage in Quality Improvement activities, staff training, and meetings.
Requirements
- Valid Physical Therapy license in California or proof of license.
- Master's degree in Physical Therapy or a Master's degree in a Physical Therapy-related field plus a bachelor's degree in Physical Therapy.
- Basic Life Support (BLS)/First Aid Certification.
- Current driver's license and proof of auto insurance.
- Two years of experience in a clinical setting with a frail or elderly population.
- Physical therapy knowledge and skills necessary to treat frail, elderly participants with complex needs.
- Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations.
- Excellent clinical, organizational, and communication skills in settings with seniors, their families, and interdisciplinary team members.
- Experience and competency working with people from diverse backgrounds.
- Commitment to unlocking the full potential of our most vulnerable seniors.
