Responsibilities

  • Evaluate residents' physical abilities, limitations, and rehabilitation needs.
  • Develop individualized treatment plans based on physician orders and clinical assessments.
  • Provide therapeutic interventions to improve strength, balance, mobility, endurance, and functional independence.
  • Assess and recommend appropriate assistive devices and adaptive equipment.
  • Educate residents, families, and caregivers on treatment plans, safety, and home exercise programs.
  • Maintain accurate and timely documentation in accordance with facility policies, Medicare, Medicaid, and state and federal regulations.
  • Participate in interdisciplinary care plan meetings and discharge planning.
  • Ensure compliance with all infection control, safety, and regulatory standards.
  • Collaborate with the rehabilitation team to achieve positive clinical outcomes.

Qualifications

  • Current New York State license as a Physical Therapist.
  • Graduate of an accredited Physical Therapy program.
  • Previous experience in a skilled nursing facility or long-term care setting is preferred but not required.
  • Knowledge of Medicare, Medicaid, PDPM, and rehabilitation documentation is preferred.
  • Excellent communication, organizational, and clinical skills.
  • Ability to work effectively as part of an interdisciplinary healthcare team.
  • Compassionate, patient-centered approach to care.
