Responsibilities
- Evaluate residents' physical abilities, limitations, and rehabilitation needs.
- Develop individualized treatment plans based on physician orders and clinical assessments.
- Provide therapeutic interventions to improve strength, balance, mobility, endurance, and functional independence.
- Assess and recommend appropriate assistive devices and adaptive equipment.
- Educate residents, families, and caregivers on treatment plans, safety, and home exercise programs.
- Maintain accurate and timely documentation in accordance with facility policies, Medicare, Medicaid, and state and federal regulations.
- Participate in interdisciplinary care plan meetings and discharge planning.
- Ensure compliance with all infection control, safety, and regulatory standards.
- Collaborate with the rehabilitation team to achieve positive clinical outcomes.
Qualifications
- Current New York State license as a Physical Therapist.
- Graduate of an accredited Physical Therapy program.
- Previous experience in a skilled nursing facility or long-term care setting is preferred but not required.
- Knowledge of Medicare, Medicaid, PDPM, and rehabilitation documentation is preferred.
- Excellent communication, organizational, and clinical skills.
- Ability to work effectively as part of an interdisciplinary healthcare team.
