About the role

Pleasant Hill Post Acute is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our rehabilitation team.

Responsibilities

  • Provide physical therapy treatments and interventions as directed by the supervising Physical Therapist.
  • Complete timely, accurate, and compliant documentation in accordance with facility policies, governmental regulations, and third-party payer requirements.
  • Maintain a safe treatment environment by adhering to infection control, safety, and equipment maintenance protocols.
  • Participate in Resident Care Conferences and Rehabilitation Department meetings as needed.
  • Aid the nursing department with training and support of Restorative Aides.
  • Communicate regularly with the supervising Physical Therapist to ensure timely updates regarding resident progress, goals, physician communication, discharge planning, and interdisciplinary care coordination.
  • Provide in-service training and educational programs for facility staff as needed.
  • Educate residents, families, and caregivers on treatment plans, home exercise programs, safety techniques, maintenance programs, and fall prevention strategies.
  • Recommend and assist with obtaining durable medical equipment to promote resident independence and safety, including walkers, canes, splints, elevated toilet seats, grab bars, and other adaptive equipment.
  • Report equipment issues promptly to ensure all rehabilitation equipment remains in safe working condition.
  • Participate in quality assurance, performance improvement initiatives, and rehabilitation program development.
  • Build positive, professional relationships with residents, families, staff members, and healthcare professionals.

Qualifications

  • Education & Experience: Associate Degree from an accredited Physical Therapist Assistant (PTA) program required. Current Physical Therapist Assistant license in the State of California required. Experience in a skilled nursing facility, post-acute care, rehabilitation, or long-term care setting preferred.
  • Experience: Proficiency with electronic medical record systems and computer applications, including Net Health and PointClickCare. Strong communication, organizational, and interpersonal skills. Ability to work effectively as part of an interdisciplinary team while providing compassionate resident-centered care.

Pay

Pay: $47–$50 per hour (Depending on Experience)

Schedule

Schedule: Monday–Friday, Full-Time
