Location Requirements

Must live in: San Francisco, CA; Antioch, CA; Tracy, CA; Oakland, CA or Sacramento, CA. Sacramento, Brentwood, San Leandro or Hayward areas preferred.

This role will cover facilities from San Jose to Oakland weekly and facilities in Sacramento monthly (or vice versa), depending on candidate home location, with compensated mileage. Physician Assistants are also welcome to apply.

Responsibilities

  • Serve as the clinical point of contact for all Providers in your region
  • Provide clinical training to ensure all Providers display excellent clinical decision making and bedside manner
  • Conduct time shadowing for each Provider in the region
  • Participate in regular clinical and facility meetings with facility staff
  • Perform focused assessments and develop individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery
  • Provide targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient-specific needs
  • Support physiatrists on-site and operate with a high level of autonomy
  • Coordinate care and refer to other specialties or services for issues beyond the scope of physiatry

Qualifications

  • 5+ years as a Nurse Practitioner (NP) or Physician Assistant (PA)
  • Current, unrestricted license as a Nurse Practitioner (NP) or Physician Assistant (PA)
  • DEA licensed or willing to obtain DEA licensure
  • Skilled Nursing Facility experience
  • Physiatry, Pain Management or Physical Medicine & Rehabilitation experience
  • Comfort in performing ultrasound guided peripheral joint injections (knees, shoulders, hips)
  • A genuine passion for providing professional, personalized care in an autonomous environment
  • Strong relationship and value proposition building with key stakeholders
  • Excellent communication skills, with the ability to collaborate with multidisciplinary teams
  • Experience in a clinical leadership or supervisory role
  • Willingness to travel regularly within the assigned region
