Lead Primary Care Nurse Practitioner

About the role

This Nurse Practitioner will be the first hire in a pioneering program at DaVita, focusing on primary care for patients with end-stage renal disease (ESKD) directly within dialysis centers. The program aims to address care barriers faced by kidney patients and bring care closer to where they spend the most time.

Responsibilities

• Provide care in the dialysis unit, reducing patient transportation needs.
• Foster seamless coordination with nephrologists, dialysis teammates, and other critical caretakers.
• Coordinate patient follow-ups to proactively address concerns and manage care with other providers.
• Build clinical relationships with various healthcare professionals and local primary care providers.
• Conduct annual comprehensive health evaluations.

Requirements

• 5+ years of independent primary care practice.
• License to practice autonomously and DEA license.
• Experience with dialysis or nephrology (preferred).
• Expertise in managing chronic health conditions, including diabetes, hypertension, and heart disease.
• Proven experience in prescribing and managing medications, including insulin, antihypertensive drugs, and other evidence-based treatments.

Qualifications

• Minimum of 5 years of clinical experience in Primary Care, Internal Medicine, or Gerontology.
• Demonstrated expertise in the management of chronic health conditions.
• Comfort in prescribing for chronic diseases typically associated with ESRD.

• Ability to work independently and as part of a multidisciplinary team.

Pay

The NP will receive a competitive base salary, with additional compensation for meeting pre-determined clinical milestones. The salary range is $133,000.00 - $164,000.00/year, with location-specific minimum wage details available upon request.

Schedule

Your schedule will revolve around the time patients will come into the dialysis units, primarily between 8:00 AM and 5:00 PM. You will have clinical autonomy to determine which patients to see and during which time.
