CLINICAL LEAD - NURSE PRACTITIONER

About the role

The Clinical Lead - Nurse Practitioner (NP) will work in collaboration with TeamCare primary care physicians to ensure proper oversight and coordination of patient care.

Responsibilities

  • Gather detailed patient histories, perform physical exams, and conduct relevant diagnostic tests.
  • Diagnose and treat common medical conditions within scope of practice.
  • Manage patients with chronic illnesses and tailor treatment plans accordingly.
  • Educate patients and families about disease prevention, health promotion, and slow progression of chronic illnesses.
  • Refer to specialists when medically warranted.
  • Document accurate and complete patient records in the Electronic Health Records (Athena).
  • Adhere to professional standards of conduct and participate in quality improvement initiatives.
  • Work effectively within a healthcare team including professionals, colleagues, patients, and families.
  • Serve adult patients at a TeamCare site, representing or serving as the Medical Director at their location in the absence or direction of the Medical Director.
  • Perform annual wellness assessments and routine quarterly assessments for patients with chronic medical illnesses.
  • Provide acute care services and facilitate same-day access for participants.
  • Assess and manage participants with acute changes in condition via face-to-face or telemedicine visits.
  • Conduct post-hospitalization visits within 7-14 days of discharge, including medication reconciliation, treatment planning, and specialist coordination.
  • Formulate a comprehensive diagnostic list of current and past medical conditions using clinical judgment.
  • Ensure all documentation is complete, accurate, and includes specific diagnosis codes in the EHR on the same day of the visit.
  • Review participants' current symptoms and identify active diagnoses and chronic problems for medical management.
  • Act as the direct supervisor of care plan formulation and delivery for community PCP participants, tailoring interventions to individual needs.
  • Act only within the scope of individual authority to practice.
  • Meet standardized competencies established by TeamCare before working independently.
  • Perform other duties as assigned.

Requirements

  • Graduated from a nurse practitioner education program acceptable to NYSED or certified as a nurse practitioner by a national certifying organization acceptable to NYSED.
  • Minimum of two (2) years in clinical nursing practice in a primary care setting, home care, care management, or related field.
  • Minimum of one (1) year of experience working with an adult population (or receive appropriate training if less than one year).
  • Effective oral and written communication and interpersonal skills.
  • High-level analytical and research skills.
  • Ability to manage multiple projects and meet deadlines.
  • Basic computer skills in Microsoft Office.

Qualifications

  • Licensed to practice as a Nurse Practitioner in NY.
  • Licensed to practice as a Registered Nurse in NY.
  • DEA Certification required.
  • AANP or ANCC Board Certification required.
  • Legally authorized to practice in the State of New York.
  • Medically cleared for communicable diseases and up-to-date immunizations before engaging in direct participant contact.
  • Must have a Medicaid Provider ID and a Medicare Provider ID in good standing.
  • Bilingual preferred.

Physical Requirements

  • Standing for up to 6 hours a day.
  • Sitting/stationary positions for up to 6-8 hours a day.
  • Lifting, pushing, or pulling up to 50 pounds of equipment, supplies, or patients safely using OSHA guidelines.
  • Bending or squatting to care for patients and use medical supplies.
  • Maneuvering stairs, walking, or climbing to access work areas throughout the day.
  • Demonstrating agility and fine motor skills to operate medical equipment and tools.
  • Visual acuity to assess patients, read orders, and document accurately.
  • Auditory and language motor skills to listen, document, and communicate clearly with patients and providers.
  • Cognitive ability for decision-making, reasoning, and analysis.

Pay

Salary ranges based on experience and bilingual ability:

  • Bilingual:
    • 1 – 5 years of experience: $170,000.00 - $175,000.00
    • 6 – 9 years of experience: $175,100.00 - $185,000.00
    • 10+ years of experience: $185,100.00 - $190,000.00
  • Non-Bilingual:
    • 1 – 5 years of experience: $162,000.00 - $165,000.00
    • 6 – 9 years of experience: $165,100.00 - $175,000.00
    • 10+ years of experience: $175,100.00 - $180,000.00

Schedule

Monday – Friday, 8:00 AM – 5:00 PM

Weekly Hours: 40
