Essential Job Functions
- Manages Health Plan-specific reports that impact medication adherence, medication reconciliation, and other STARs and HEDIS measures.
- Affords assistance and training to the team in maintaining high levels of patient satisfaction by providing exceptional customer service and acting as the liaison between patients, physicians, and pharmacies.
- Oversees and manages daily, weekly, monthly, and open item activities within the Pharmacy Department to ensure smooth operations.
- Develops and delivers training materials to the Pharmacy Department team as necessary.
- Assists in managing members' care within chronic disease management programs through outreach to members, providers, and vendors.
- Engages in member and provider outreach to enhance Part D quality initiatives, tracking outcomes through data entry.
- Supports healthcare professionals at various levels in matters related to patient medications.
- Phones and faxes for provider office outreach to confirm prescriptions and other required data.
- Provides education to primary care physician offices and staff regarding Part D STAR measures.
- Supports clinical pharmacists in Medication Therapy Management for Health Plan members.
