About the role

Responsible for the provision of pharmacist care by working in collaboration with multidisciplinary healthcare teams to ensure quality care, education and research, and patient safety. Works to promote guidelines and policies and assists in the development of programmatic changes to improve the care provided to patients throughout the health system. Provides education to patients, providers, students, residents, and others. Assumes a leadership role in the professions of pharmacy and quality improvement.

Responsibilities

  • Interacts with third party payers to ensure adherence to measures.
  • Ensures patient compliance and minimize risk associated with high risk medications through a variety of techniques including appropriate reminder devices, patient’s calls, and home visits.
  • Provides data on medication use to providers.
  • Routinely participates in the care of patients.
  • Involved in consultation with care providers regarding the optimal care for patients.
  • Serves as a resource for care providers regarding guidelines and best practices.
  • Interacts with care providers to ensure quality care is provided.
  • Consistently provides latest information regarding guideline development and best practice alerts to clinical staff throughout enterprise.
  • Collections data and submits as required by various pay-for-performance initiatives and quality reporting requirements.
  • Ensures accuracy and transparency of data.
  • Prepares and participates in presentations as required by various pay-for-performance programs.
  • Participates in the educational efforts of the Center for Quality Outcomes, the WVU School of Pharmacy, and the WVU School of Medicine.
  • Provides ACPE, ACGME, and Nursing continuing education programs throughout year.
  • Provides lectures and other presentations as requested.
  • Participates in the development of research projects.
  • Conducts and leads research projects in quality and safety arenas.
  • Assists in the Medication Use Evaluation process in the Department of Pharmaceutical Services.

Qualifications

  • Minimum Qualifications: Must possess current licensure as required by state board where services will be provided: MD: Licensed Pharmacist through Maryland Board of Pharmacy OH: Licensed Pharmacist through Ohio Board of Pharmacy PA: Licensed Pharmacist through Pennsylvania Board of Pharmacy WV: Licensed Pharmacist through West Virginia Board of Pharmacy Remote Workers: Must additionally hold current Pharmacist license issued by state of residence.
  • Preferred Qualifications: Advanced degree (Master of Public Health or other degree). Experience Residency training OR Three (3) years of pharmacist experience.
