Accountabilities
- Support medication safety, quality improvement programs, and clinical pharmacy initiatives.
- Partner with healthcare providers, patients, and multidisciplinary teams to ensure effective medication management, promote best practices, and contribute to research and education efforts.
- Implement pharmacy guidelines, policies, and quality improvement initiatives.
- Support the implementation of pharmacy guidelines, policies, and quality improvement initiatives.
- Work with third-party payers and healthcare stakeholders to support adherence to quality measures.
- Promote medication adherence and reduce risks associated with high-risk medications through patient outreach, education, and support strategies.
- Provide medication-related data and insights to healthcare providers to support clinical decision-making.
- Complete medication reconciliation activities for selected patients transitioning to post-acute care settings.
- Serve as a pharmacy resource for providers regarding clinical guidelines, best practices, and medication management strategies.
- Monitor, collect, and report quality data required for performance programs and regulatory initiatives.
- Ensure accuracy, transparency, and reliability of quality reporting information.
- Prepare and deliver presentations, educational sessions, and continuing education programs for healthcare professionals.
- Participate in research projects focused on medication use, quality improvement, and patient safety.
- Support medication use evaluations and other pharmacy service improvement initiatives.
Requirements
- A licensed pharmacist with experience in clinical pharmacy, healthcare quality, or medication management.
- Strong communication skills, analytical thinking, and the ability to collaborate effectively with multidisciplinary teams.
- Current pharmacist license in the state where services are provided.
- Residency training or at least 3 years of pharmacist experience preferred.
- Advanced degree such as a Master of Public Health or another relevant graduate qualification preferred.
- Strong knowledge of pharmacy practices, medication safety principles, and healthcare quality improvement.
- Ability to interpret clinical guidelines, analyze data, and support evidence-based decision-making.
- Excellent verbal and written communication skills.
- Strong teamwork, collaboration, and relationship-building abilities.
- Effective time management skills with the ability to prioritize multiple initiatives.
