Personal Care Outreach Coordinator

About the role

We are looking for a Personal Care Outreach Coordinator (SALES) to join our team. This position will directly report to the Assistant Director PC or Branch Director PC and is responsible for overseeing client referrals, scheduling and performing assessments, and marketing to referral sources. Responsibilities include overseeing VA and Private Duty client referrals, assisting Veterans and families in navigating the VA system and LTC Insurance, performing internal and external marketing, and completing weekly statistical reports pertaining to intake and discharges.

Qualifications

• High school graduate; some college preferred
• 2‑4 years’ experience in sales, marketing, customer service preferred
• Prior experience in healthcare

Skills

• Excellent professional communication and organizational skills
• Bilingual preferred (Spanish)
