About the role
Join our team as a Wellness Coordinator, where your outreach and care coordination will directly support patients in accessing vital preventive services and improving their overall well-being.
Responsibilities
- Make outreach calls to patients, guiding them through eligible preventive care services like Medicare Annual Wellness Visits.
- Educate patients on the importance of preventive care and help close care gaps that impact long-term health.
- Work side-by-side with physicians, nurses, and staff to create a seamless, integrated approach to patient care.
- Support primary care practices in achieving better outcomes through documentation, process enhancements, and occasional presentations at staff meetings.
- Assist with general office tasks, secretarial duties, and population health initiatives that make a real difference in our communities.
Requirements
- A positive attitude and a team-first mindset.
- Strong communication skills—especially over the phone and via text.
- Comfort with technology, including Microsoft Office and electronic medical records.
- Ability to multi-task, prioritize, and stay organized in a fast-paced environment.
- A basic understanding of medical terminology is a plus.
Qualifications
- Education: High school diploma or equivalent required; Associate Degree preferred.
- Experience: Prior clerical experience in a healthcare setting is preferred.
