Pediatric Team Lead Pharmacist

Description

This position exists to provide comprehensive, value based, medication-related care to achieve definite outcomes that improve a patient's quality of life. The incumbent acts in collaboration with other healthcare professionals in the shared responsibility for patient drug therapy outcomes.

  • Ensuring the safety and effectiveness of the medication use process
  • Processing and dispensing medications per order or prescription
  • Medication therapy monitoring
  • Regulatory compliance
  • Maintaining oversight over pharmacist extenders
  • Maintaining the security of medications
  • Protecting the confidentiality of patient information

Key job duties

Key job duties may consist of:

  • Ensuring the safety and effectiveness of the medication use process

  • Maintaining the security of medications
  • Protecting the confidentiality of patient information

Qualifications

Requires Doctor of Pharmacy (PharmD) or Bachelor's of Science in Pharmacy prior to 2006. Requires current Indiana License or eligibility for Indiana licensure as a pharmacist. Certification is required, if applicable. (e.g. BCPS).

Demonstration of > 5 years experience, pharmacy residency training, or advanced degree is required.

Demonstration of leadership attributes.
