About the role
Imagine Pediatrics is a tech-enabled, pediatrician-led medical group offering 24/7 virtual and in-home medical, behavioral, and social care. We work alongside families, providers, and health plans to enhance existing care teams with support, compassion, creativity, and a commitment to children with medical complexity.
Responsibilities
- Provide medication management services for pediatric patients with chronic medical and behavioral health conditions, focusing on optimizing medication therapy, disease outcomes, and value of care.
- Support medication access management services between the patient, pharmacy, insurance, and medical provider through oversight and support of the region Medication Support Navigator.
- Offer accurate, thorough medication education, adherence assessment, and comprehensive/directed medication management through direct interactions with patients/caregivers, internal and external care team members, and independently under individualized scope of clinical practice (where applicable).
- Document encounters in the patient electronic medical record, including closed-loop communication with the patient care team.
- Promote evidence-based, cost-effective medication management by staying updated on new drugs, devices, and medical literature.
- Document and report adverse drug events (ADEs), medication allergies, and medication errors according to procedures.
- Liaise between the designated region and the medication management center of excellence.
- Practice pharmacy in compliance with all governing regulatory authorities.
- Perform other duties as assigned.
Qualifications
- Graduate of an accredited college/university of pharmacy (Pharm.D)
- Unrestricted, current pharmacist license with ability to obtain licensure by reciprocity in each state where telehealth services will be provided
- Active license in Washington or California required
- Completion of ASHP accredited PGY-1 Pharmacy Residency, preferred settings include pediatric or ambulatory care
- Completion of an ASHP accredited PGY-2 Pediatric or Ambulatory Care Pharmacy Residency, preferred
- Board Certified Pediatric Pharmacy Specialist (BCPPS), Board Certified Pharmacotherapy Specialist (BCPS), OR Board-Certified Ambulatory Care Pharmacists (BCACP), preferred
- Applicants who are not board-certified will need to be eligible for board-certification in a preferred specialty area and will be expected to pursue and successfully achieve certification within an agreed upon period
- Bilingual English/Spanish preferred
- 2+ years of practice experience in pediatrics, post-PGY1 residency
- Pediatric or ambulatory care setting strongly preferred
