Clinical Pharmacist Pediatric

Essential Functions

• Reviews patients' medication order for allergy interactions, duplication of therapy, appropriateness of drug therapy, and drug-drug interaction.
• Makes appropriate evidence-based interventions to optimize medication use.
• Safeguards and efficiently delivers medications to care areas.
• Promotes quality medication education and adherence.
• Counsels patients and ensures they are well-informed, treated respectfully, and receive high-quality service.

Minimum Qualifications

• Doctor of Pharmacy or Bachelor of Science in Pharmacy degree from an accredited institution (education verified).
• Licenses to practice pharmacy in the state of practice and dispense controlled substances.
• Current knowledge of state and federal regulations and statutes regarding the practice of pharmacy in the state of practice.
• Effective communication, people management, and project management skills.
• Immunization certification if applicable to practice site needs.

Preferred Qualifications

• Pediatric hospital pharmacy experience.
• Previous experience working in an integrated healthcare delivery system.
• Board Certification.
• Health System Residency.

Physical Requirements

• Ongoing need to see and read information, labels, documents, monitors, identify equipment and supplies, and assess customer needs.
• Frequent interactions with providers, colleagues, customers, patients/clients, and visitors requiring verbal communication, hearing, and understanding of spoken information, needs, and issues quickly and accurately.
• Manual dexterity of hands and fingers to manipulate complex and delicate supplies and equipment with precision and accuracy.
• Frequent computer use for typing, accessing needed information, etc.
