Pediatric Palliative Care Specialist- RN, Full-time (Sign-On Bonus Available)

About the Role

This is a full-time professional position as part of the Transitions Kids (TK) interdisciplinary team. The role involves developing and implementing care plans and interventions based on patient and family goals, in coordination with the care team. This position includes after-hours coverage, telehealth visits, and home visits to provide symptom relief and support during assigned shifts.

Responsibilities

• Provide medical and emotional support for families across the continuum of care, including during times of crisis.
• Collaborate with the Interdisciplinary Group (IDG) to assess, establish, and support patient/family-centered care goals; determine the patient’s level of care; and coordinate service utilization.
• Use professional judgment, in collaboration with the IDG, to individualize the plan of care based on the patient and family’s unique needs.
• Provide immediate and long-term assessment and documentation of the needs of patients and families enrolled in the TK program.
• Plan, provide, supervise, and document safe, competent, and compassionate care in accordance with physician orders and established policies.
• Collaborate with the patient, family, facility staff, and TK team to develop culturally sensitive care plan components.
• Identify and make referrals to appropriate community organizations.
• Provide and document end-of-life education to facility staff, patients, and families in compliance with CMS Conditions of Participation (CoPs).
• Monitor and coordinate the appropriate use of patients’ medical equipment, supplies, and medications with the Administrative Team.
• Delegate tasks and supervise the activities of LPNs and Hospice Aides, as needed, following CoPs for CNA supervision.
• Provide after-hours phone coverage and home visits as arranged with a supervisor.
• Contact DME companies, pharmacies, transportation services, and other community entities involved in patient care as needed.
• Participate in Transitions Kids team meetings and professional development activities.
• Actively participate in the orientation and training of new employees and students.
• Maintain current training and knowledge of pediatric nursing practices and pediatric hospice/palliative care.
• Perform other duties as assigned by the Transitions Kids Director or Team Lead.

Requirements

• Registered Nurse with a current NC license; BSN required.
• Minimum of 2 years of recent pediatric clinical experience; pediatric hospice, hospital inpatient, or home health experience preferred.
• Pediatric Hospice and Palliative Care Nurse Certification (CHPPN) preferred.
• Bilingual (particularly Spanish) preferred.
• Able to build trusting relationships with patients, families, and coworkers.
• Able to work independently and collaboratively within a team.
• Strong communication skills with patients, families, IDG team, physicians, facility staff, and leadership.
• Experience in goals-of-care conversations and advance directives preferred.
• Ability to educate patients and families in accordance with the plan of care.
• Knowledge of reimbursement sources, Medicaid regulations, licensing laws, and accreditation standards.
• Competent computer skills with the ability to learn new software programs.
• Current CPR certification.
• Valid North Carolina Driver’s License, automobile insurance, and reliable transportation.
• Ability to travel to and function in patient homes/facilities within the service area.
• May be required to lift and/or carry items up to 50 pounds.
• Sensory abilities to complete physical assessments, communicate effectively, and document care.

Schedule

Monday–Friday, 8:30 AM–5:00 PM, with PRN weekend and holiday commitments as outlined in the Employee Information Agreement.
